Using the site

To see the list of forms we have available click on the Forms button on the toolbar.


Before starting to fill in a form we recommend that you sign in. This will allow any previously saved answers to be retrieved and will allow the system to email you your completed form.




If you are new to the site you can Sign Up using the link on the Sign In screen.




Signing up is free. You just need to supply your email address and make up a password. During the Sign up process we will send you a verification email to confirm you have given us your correct email address. Just click on the link in the email (or copy and paste it into your browser) to finalise the verification process.

You can find help relating to the form you are working on by clicking the Help link in each questionnaire.




When you have finished completing the questionnaire click on the Submit button on the toolbar and your completed form will be emailed to you. This generally only takes a minute or two.

If you don't have time to complete a questionnaire you can save your answers for later by clicking the Save button on the toolbar. Saved answers are automatically loaded into your form when the form loads. To remove your answers from the system click the Delete button.
